Gummy Bear Breast Implants: VIDEO

Many people still believe that saline breast implants are a safer choice over silicone, but in many cases this belief is based on old information about outdated implant technology.

Now that highly-cohesive silicone gel implants (AKA Gummy Bear Implants) are FDA-approved for cosmetic breast augmentations, comparing saline to silicone is like comparing apples to oranges. When silicone implants were made with a liquid silicone core in the 1970’s, leaks were a real concern, and it made sense that perhaps a saline implant might be a better choice.

Nowadays, the majority of silicone breast implants are manufactured with a highly-cohesive silicone gel core, and therefore leaks are essentially a thing of the past. Because of this, the choice between saline and silicone has become much more straightforward. The gel core in the newest generation silicone implants is “cohesive” which means that it sticks to itself and does not travel away from its own silicone core. The above video shows a highly-cohesive gel implant that has been physically cut in half with a scalpel. As you can see, the silicone gel is a solid gel that doesn’t leak out of the implant shell even when the shell has been cut. And even though this gel is a solid, it is still incredibly soft so this implant feels very natural after surgery.

Inverted Nipple Repair: The easiest procedure you never knew about!

Inverted nipples are pretty easy to find in the general population, but surgeons who fix them aren’t as common. Most women don’t even know that there is a procedure to repair inverted nipples, so we thought this Before & After photo might be “worth a thousand words”.

An inverted nipple repair procedure can be done under local anesthesia, and takes about 90 minutes to do both sides. You can go back to work the same day if you have to, but most patients will opt to go back the following day. The recovery period is pretty painless (most patients will only take a few prescription strength pain pills the night after surgery and then switch to regular Tylenol), and most of swelling will be gone in less than a week.

The After photo in the above image is the result at 2 weeks after surgery, and you can see that her nipple has gone from inverted to a normal appearance with almost no visible scars.

At first glance this may seem like an easy surgery, but this is actually one of the toughest breast augmentations: a very petite woman who is in very good shape and wants a large size implant. It is extremely difficult to achieve a natural look with this combination of factors, but as you can see, it is possible. A few months ago, I had the opportunity to treat a woman who traveled from Washington DC to have Botox therapy for her very severe migraines. She had been treated previously in California, Canada, and in several other locations, and although she experienced some relief from those treatments, she never actually “broke the migraine cycle” as I like to call it. Her headaches were less intense and slightly less frequent, but she was still tied to her oral migraine medications (and to their side effects).

Factors Affecting Costs of Plastic Surgery

The cost of cosmetic surgery in Manhattan, New York City is likely to be different from the cost in Dallas or Atlanta. Does my choice of incision affect the risk of capsular contracture after breast augmentation?

incision choices for breast augmentation surgeryNew data presented at the 2013 American Society for Plastic Surgeons (ASPS) meeting suggests that your choice of incision does affect your risk of capsular contracture.

The overall risk of capsular contracture after breast augmentation surgery is a range of about 15-20%, but new data suggests that the risk can be decreased to approximately 5% with inframammary incisions (in the fold under the breast). The two other most common incisions for breast augmentation are the periareolar incision (around the bottom half of the areola) and the transaxillary incision (through the armpit). This new research suggests that you have a significantly reduced risk of capsular contrature by choosing an inframammary incision for your breast augmentation procedure.

IPL for Freckles and Sun Damage

IPL treatments for summer sun damage and freckles

Intense Pulsed Light (IPL) therapy is a fantastic way to reduce the number of freckles and sun damage on your skin, so now that fall is officially here, and winter is right around the corner, consider a series of 3-4 treatments to get your skin ready for the next sunshine season.

The woman in the above photo series had two fairly aggressive treatments with the MaxG IPL/OPL system from Palomar, and you can see the amazing difference in the uniformity of her skin tone. The treatments were spaced about 4 weeks apart, and she did elect for a macrodermabrasion treatment in between her IPL sessions to speed up the process of the freckles flaking off. She had several freckles and flat moles that got extremely dark in the summer sun, and that is what prompted her to seek treatment.

The treatments themselves are fairly painless with numbing cream applied to the skin beforehand, and the treated areas will be bright pink for a few hours. The freckles themselves will turn even darker almost instantly after the treatment, and will continue to darken over the following 24-48 hours. This is a sign that the treatment was effective… the light was effectively absorbed by the pigment, and the pigment has changed in response to the light. If you don’t see this darkening, chances are that your treatment was either too weak, that the machine isn’t the best technology, or that your laser specialist didn’t customize the settings properly to your particular skin type.

The dark spots will begin to flake off about 3-4 days after the treatment, and most of the smaller and lighter spots may be completely gone after a week or so. The largest and darkest spots may take a few weeks to fade completely, and the most stubborn of those may need a few separate treatments.

Earlobe Repair Surgery

The most common reason for earlobe repair is a gradual weakening of the earlobe tissue caused by heavy earrings. It can also be caused by catching your earring on something and forcefully tearing the lobe, but showing off heavy bling is the most likely culprit.

The good news is that torn and damaged earlobes can be repaired. Earlobe repair is a fairly pain-free procedure that can be done under local anesthesia in the office setting. For most patients, it takes about an hour, and you can go back to work the same day if necessary (although we do recommend that you do take the day of the procedure off so you can apply some ice).

The damaged section of the earlobe is excised, and the remaining healthy tissue is meticulously repaired with very delicate stitches on the inside and out. The stitches are so small that most people would not notice anything out of the ordinary about your repaired lobes when you go back to work, and they don’t require any special care other than keeping them clean with your normal face wash. The outside sutures are removed on the 5th or 6th day after the procedure, and the inside stitches dissolve after earlobe tissue is healed. The incision does leave a small, but very inconspicuous, scar that will be hidden almost completely after the ear is re-pierced. The scar will be red/pink for several weeks after the procedure, and then will gradually fade to an almost invisible thin line.

A meticulous repair involving the internal and external tissues of the lobe is the key to a strong, long-lasting repair, so be cautious about who performs your procedure. The surgeon should be comfortable with earlobe surgery, and he or she should be willing to take their time to ensure an adequate repair. Otherwise, the lobe will simple tear through again as soon as you start to wear earrings.

Women can re-pierce their ears and wear light earrings about 8 weeks after surgery, but clips on earrings can be worn before then.

Combination with Breast Lift

Breast augmentation as an independent procedure cannot correct severely sagging breasts. This condition may offer due to aging, skin laxity or other factors. Procedure

  •  Transaxillary incision
  •  Periareolar incision

The choice of implant size will largely depend on the breast anatomy, body type, and skin elasticity.

Following the placement of implants, the plastic surgeon will close the incisions with sutures.

Results and Recovery

Breast augmentation results can be visible almost immediately, and full effects can be seen as the post-op swelling resolves and incision lines fade. Recovery takes place within 24 to 48 hours following the procedure, but the cosmetic surgeon will recommend an additional period of reduced activity for a few days. Minor swelling and soreness may remain for a few weeks. The surgeon may provide pain relieving medication for a few days to keep the patient comfortable.
