She underwent a Liquid Facelift procedure that focused mostly on her mouth region, but also on correcting the hollowness under her eyes, and elevating her brows by relaxing the tense muscles around the eyes and forehead. The softest grade of Juvederm (Ultra) was used with a special microneedle to fill each tiny crease around her mouth, and then her lips themselves were augmented for just a bit of natural fullness and plump which smoothed out the wrinkles on the surface of her lips as well. A slightly plumper grade of Juvederm (Ultra Plus) was then used to add some soft voluime back to her undereye areas, and lift her cheeks and lower face ever so slightly. Juvederm Ultra Plus was also used to recontour the area around her chin by hiding the small jowls that were making her chin look larger and more masculine than it actually was. The last step in the proceudre was 35 units of Botox to relax her brows. This is a 31 year-old woman who wanted a smaller, more defined, and more feminine tip. Her first question during her consultation was “Can I only do the tip of my nose since that’s all I care about?”. The answer to this question is no, it is almost always a mistake to change only the tip of the nose. The nose is a very three dimensional structure, which is why rhinoplasty surgery is so difficult. Because the nose is so three dimensional, it is often necessary to make small adjustments to each part of the nose (the tip, the bridge, and the nostrils) in order to achieve the most natural result. Because the tip of her nose was reshaped to be smaller and prettier, it was also necessary to narrow her bridge just a touch so that the overall appearance of nose remained balanced and natural. This is a 30 year old woman who wasn’t happy with the appearance of her 10-year old saline breast implants, particularly the appearance of her right implant. Her right breast implant has slid nearly all the way off her chest into her armpit, and looks quite different than the opposite side. Her right implant is in such a bad position that it is actually uncomfortable for her.
Her old 330cc high profile saline implants were removed, major revisions of the implant capsules on both sides were performed, and new 457cc moderate profile gummy bear silicone implants were reinserted. This is the result at one month after surgery, and you can see the tremendous improvement in the position of her right implant. And because of the improved positioning of both implants, they actually look more natural even though they are significantly larger. This is a 33 year old woman who wasn’t happy with the appearance of her saline breast implants. Her main goals for the revision surgery were a larger size and better symmetry.
Her old saline implants were removed and found to be dramatically different sizes (275cc on her right and 200cc on her left) for an unknown reason, and they were replaced with Natrelle Inspira Style SRM-445cc smooth round silicone implants. The left side was particularly difficult because it had been so underexpanded for so many years. The muscle needed to be re-released and the skin re-expanded in several locations to allow the left implant to sit in a more symmetric spot in comparison to the opposite side. This is the result at 2 months after surgery, and not only is the symmetry dramatically improved, but the larger size actually looks more natural. This may seem a bit counterintuitive how a larger size could look more natural than a smaller size, but when the implants are positioned properly, this is commonly the case.

This is a 32 woman who was not happy with the appearnce of her old saline implants for several reasons. Firstly, she wanted a larger, sexier size without looking obviously fake, so she opted to have The Playmate Technique to upgrade her 350cc salines to 560cc silicone implants. Secondly, she wanted to improve the feel of her implants, and minimize the rippling that is very common with saline implants. In order to accomplish this goal she opted for the newest generation smooth round silicone implant with a fuller gel fill, the Inspira implant. The Inspira implant is an excellent device that combines the amazingly natural feel of the teardrop implant with the sexiness of a round implant, and it is the only implant on the market that accomplishes these two things. Thirdly, she wanted to fix her extreme animation deformity (see the last photo in the front view). When she flexed her chest muscles, her right implant moved considerably towards her armpit and took on a deformed shape. This is due to an inadequate release of the perctoralis muscle fibers during her original breast augmentation surgery, and it was repaired with a meticulous revision technique during her Playmate operation.

This is a 39 year old woman who had a breast augmentation performed by another surgeon and was unhappy with the implants sitting too high on her chest, the square rectangular shape of the implants, the uneven appearance between the two sides, and the fact that her breast sagging was not well corrected.

This is a 35 year old woman who elected to have her old 450cc saline implants exchanged for teardrop shaped Gummy Bear implants.
She underwent a fairly complicated breast implant revision surgery in which her old implants were removed, the capsules around her old implants were removed as well, and the new implant pockets wre completely reshaped in order to accommodate an implant with a totally different shape and texture. The implants that were removed were smooth, round, high profile 450cc saline, and they were replaced with Style 410FX-560cc teardrop gummy bear implants. An internal breast lift was performed as well, and this is an exceptionally good result given that she did not have a formal mastopexy (breast lift with anchor scars). This is a 53 year old woman whose breast implants were persistently asymmetric after several operations by her previous surgeon.
She had a breast augmentation many years ago that did not produce enough of a lift because the implants were too small, and then underwent an exchange to a larger size along with a mastopexy (breast lift) a year later. She unfortunately developed a capsular contracture on her left side after the first surgery, and it recurred after her second operation as well.
This time around, she underwent an extremely complicated revisional surgery where her implants were increased from 305cc to Natrelle Inspira 560cc in order to more appropriately volumize the breast without having to redo the mastopexy, and her left sided capsular contracture was repaired by performing a full capsulectomy (removal of the scar tissue around the implant) and using a dermal matrix product called Alloderm to reduce the risk of a third recurrence.
This is the result at 8 months after surgery, and so far there is no evidence of recurrence of her contracture, her breasts are fuller and more appopriately lifted, and although her left breast still has 9-12 months of settling to do, the result is still quite an improvement.

This is a 40 year old woman whose breast implants were markedly asymmetric after several operations by her previous surgeon. The left implant and breast were sagging to the point of causing physical discomfort and an inability to wear the clothing she wanted to wear.
She underwent an extremely complex breast implant revision procedure in which her original implants were removed, the implant capsules were restructured to allow the new implants to sit in a more natural position, and the both capsules were further supported with a sling of SERI Silk Mesh Scaffold to create what is essentially an internal bra. A new set of 700cc smooth round moderate profile implants were then inserted, and this is the result at at approximately 2 months after surgery. In her After photo she is still wearing silicone strips to fade and protect her scars, but if you look past the visiblle silicone strips the improvement is quite dramatic. The discomfort in her left breast is gone, and she can wear any clothing she desires comfortably.
If you look closely you will see a few irregularities at the bottom of her left breast. This is normal and expected at such an early stage after a complex revision like this, and is a result of the natural nealing process of the SERI Scaffold directly under the skin supporting the new implants. These irregularities will improve over the next several months, and the left implant will settle into a more natural position and round out beautifully on the bottom edge as the SERI stretches.

This is a 24 year old woman who has her original implants done in Miami by a “prominent” surgeon, but was unhappy with them for obvious reasons. The Before picture is her result one month after her original surgery in Miami, and as you can see, her implants are far too high and have a terribly unattractive rectangular appearance. If you look closely, you’ll also notice that her scars are puckered and very noticeable even though they have had more than a month to heal.

Her old implants were not only too small for her frame, but they were also asymmetric with her left breast being particularly high and contracted in the lower pole. This procedure was particularly difficult in attempting to improve the symmetry, and the left side was found to have a scarred implant capsule that needed to be removed to make space for the new implant, and to allow the implant to sit in a more natural position. Although the symmetry is still not as good as it could have been if she had had a better original breast augmentation, it is significantly improved, as is the overall size, shape, and proportion.

This is a 55 year old woman who was interested in having her 16 year old breast implants exchanged for a new pair because she was incredibly unhappy with how they looked, and because she was developing a capsular contracture (hardening) of her left implant. She noticed that her left breast started getting hard and changing shape about a year prior to her initial consultation, but she put it off because she wasn’t sure if it was a problem. This ended up being a mistake becuase the extra time that she waited allowed her contracture to evolve to a stage that was very difficult to fix, and during the revision surgery, her left silicone breast implant was found to be ruptured. The ruptured implant material had to be removed, the hardened scar tissue had to be removed, both breast pockets had to be reshaped, and new 421cc moderate profile silicone implants were replaced for a much more natural look that is also much softer after the contracture repair.
This case illustrates the tremendous importance of planning to exchange your breast implants for a new pair at the 10-15 year mark for silicone, or the 6-8 year mark for saline. It also illustrates the reason that it’s important to seek the help of a plastic surgeon right away if your breast implants become hard or change shape. This could be a sign of a serious problem like capsular contracture, and the quicker you have it dealt with, the easier the repair procedure will be.
